Self-written Biographical sketch.<\/p>\n<p>4. Undertaking letter not to perform other than the religious matters.<\/p>\n<p>5. Sponsor letter from the Meditation Center\/ Monastery\/ Venerable<br \/>\nMonk in Myanmar.<\/p>\n<p>6. Two<a href=\"http:\/\/myanmarbsb.org\/_site\/wp-content\/uploads\/2018\/06\/religious_visa_form.pdf\">\u00a0Religious Visa Forms<\/a>\u00a0with photos, both filled with pen (originals).<\/p>\n<p>7. One extra photo (5&#215;7 size) to be delivered with the documents.<\/p>\n<p>8.
